New Students
8 a.m. – 4 p.m.
Donna Rushing, Maple Grove secretary, will be at the elementary school to register new students. Kindergarten students who did not pre-register in the Spring, should contact Donna Rushing or Landon Sommer, principal, at 618-543-7434 prior to Friday, August 7th.

Existing students
8 a.m. – 9:30 a.m.
Buses will run and registration will be on this first day of school. Schools will run on a half day schedule the first day with early dismissal at noon.


JR. HIGH/SR. HIGH SCHOOL

Monday-Friday, August 3-7
New Students
9 a.m. – 3 p.m.
Penny Bellamy, guidance counselor, will be at the Jr. High/Sr. High School to register new students, check schedules, and make recommendations. Records will be requested from previous school. Transfer students will be placed on a schedule after records are received, and credits and grades have been evaluated.

Buses will run and registration will be on this first day of school. Students will remain in their home rooms for completion of district paperwork. Schools will run on a half day schedule the first day with early dismissal at noon.


REQUIREMENTS

Physical examinations, immunization records and official birth certificates are required to be on file for all students.

Students entering kindergarten, 6th- and 9th-grades are required to have an updated physical examination and appropriate immunizations prior to entering school.

Students entering kindergarten, 2nd- and 6th-grades are required to have a dental exam prior to school starting.

Students in kindergarten, or who are attending public school for the first time, are required to have a vision exam upon entering school. All new students must provide a copy of a recent physical examination, vision exam, copy of the student immunization record and official birth certificate upon registration. Forms for physicals, dental and vision exams are available at the J-MG School District office.

Hospital birth certificate will not be accepted. Students must provide a copy of the official certificate from the state in which they were born. Students will not be allowed to attend classes until all Illinois state mandated requirements for physical, dental and vision exams, immunization, and birth certificates are met.

Students who participate in sports activities must present proof of physical exam prior to the beginning date of the sport. Sports physicals do not replace the mandatory physicals required for students entering 6th- and 9th-grades.

COSTS

Textbook rental fees are $20. Students will be charged $20 per sport per year not to exceed $50 per family. Driver’s Ed fee is $50 for the class in addition to the $20 White Slip Fee to the Secretary of State. High school art students will incur a supply fee of $10. Waivers for student fees and breakfast and lunch meals are available by completing a form and returned to the appropriate school office by the end of the first two weeks of attendance.
